Seniorcorp is hiring a Registered Nurse to serve as a nurse case manager, working closely with new and existing Seniorcorp clients and their families.
The nurse case manager develops, implements, and evaluates individualized care plans, acts as a patient advocate, and serves as a liaison between patients, their families and other healthcare providers to meet the needs of the individual in the home and community setting.
This position has some time devoted to supporting our home care clinical team in developing customized care plans for new clients beginning service, visiting existing clients periodically to review their care needs, and supporting and assisting our caregivers in the delivery of care. The case management work will become a larger part of the role as that part of the business grows.
At least five years clinical experience in nursing is required. Hospice, cardiology, or oncology experience is a plus. Case management certification is helpful but not required. Important skills include:

Must be comfortable working independently, but great communication skills are also required as you will interface on a daily basis with clients, caregivers, administrative staff and our clinical team
