Full Job Description:
Will handle admission of patients. Will counsel patients to maximize their independence and employability by coping with personal, social and vocational difficulties that result from birth defects, illness, disease, accidents, or the stress of daily life. Will coordinate activities of patients needing rehab and physical therapy services. Will assess client needs, discuss their case with rehab/health care providers and obtain their plan of care for each patient. Will ensure that rehab and other health care providers implement rehabilitation services and programs that may include personal and vocational counseling, training, and preparation for job placements.
